ABSTRACT

ABSTRACT Introduction: Surgically assisted rapid palatal expansion (SARPE) has been the treatment of choice in subjects presenting skeletally mature sutures. Objective: The purpose of this study was to analyze stress distribution and displacement of the craniofacial and dentoalveolar structures resulting from three types of palatal expanders with surgical assistance using a non-linear finite element analysis. Material and Methods: Three different palatal expanders were designed: Model-I (tooth-bone-borne type containing four miniscrews), Model-II (tooth-bone-borne type containing two miniscrews), and Model-III (bone-borne type containing four miniscrews). A Le Fort I osteotomy was performed, and a total of 5.0 mm palatal expansion was simulated. Nonlinear analysis (three theory) method (geometric nonlinear theory, nonlinear contact theory, and nonlinear material methods) was used to evaluate stress and displacement of several craniofacial and dentoalveolar structures. Results: Regardless of the maxillary expander device type, surgically assisted rapid palatal expansion produces greater anterior maxillary expansion than posterior (ANS ranged from 2.675 mm to 3.444 mm, and PNS ranged from 0.522 mm to 1.721 mm); Model-I showed more parallel midpalatal suture opening pattern - PNS/ANS equal to 54%. In regards to ANS, Model-II (1.159 mm) and Model-III (1.000 mm) presented larger downward displacement than Model-I (0.343 mm). PNS displaced anteriorly more than ANS for all devices; Model-III presented the largest amount of forward displacement for PNS (1.147 mm) and ANS (1.064 mm). All three type of expanders showed similar dental displacement, and minimal craniofacial sutures separation. As expected, different maxillary expander designs produce different primary areas and levels of stresses (the bone-borne expander presented minimal stress at the teeth and the tooth-bone-borne expander with two miniscrews presented the highest). Conclusions: Based on this finite element method/finite element analysis, the results showed that different maxillary expander designs produce different primary areas and levels of stresses, minimal displacement of the craniofacial sutures, and different skeletal V-shape expansion.


RESUMO Introdução: A expansão rápida da maxila assistida cirurgicamente (ERMAC) tem sido o tratamento de escolha em indivíduos que apresentam suturas esqueleticamente maduras. Objetivo: O objetivo deste estudo foi avaliar, utilizando uma análise não linear com elementos finitos, a distribuição de tensões e os deslocamentos das estruturas craniofaciais e dentoalveolares gerados por três tipos de expansores palatinos usados na ERMAC. Material e Métodos: Três tipos de expansores palatinos foram projetados: Modelo I (dento-osseossuportado com quatro mini-implantes), Modelo II (dento-osseossuportado com dois mini-implantes) e Modelo III (osseossuportado com quatro mini-implantes). Uma osteotomia Le Fort I foi realizada e foi simulada uma expansão palatina total de 5,0 mm. Um método de análise não linear (três teorias - teoria da não-linearidade geométrica, teoria do contato não linear e métodos para materiais não lineares) foi utilizado para avaliar a tensão e o deslocamento de diversas estruturas craniofaciais e dentoalveolares. Resultados: Independentemente do tipo de aparelho expansor palatino, a ERMAC produziu maior expansão anterior da maxila do que posterior (ENA variou de 2,675 mm a 3,444 mm e ENP variou de 0,522 mm a 1,721 mm); o Modelo I apresentou padrão de abertura mais paralela da sutura palatina mediana, com ENP/ENA igual a 54%. Com relação à ENA, o Modelo II (1,159 mm) e o Modelo III (1,000 mm) apresentaram maior deslocamento para baixo do que o Modelo I (0,343 mm). A ENP deslocou-se mais para anterior do que a ENA com todos os aparelhos; o Modelo III apresentou o maior deslocamento para anterior da ENP (1,147 mm) e da ENA (1,064 mm). Os três tipos de expansores apresentaram deslocamento dentário semelhante e separação mínima das suturas craniofaciais. Como esperado, diferentes designs de expansores palatinos produzem diferentes áreas primárias e níveis de tensões (o expansor osseossuportado apresentou tensão mínima nos dentes, e o expansor dento-osseossuportado com dois mini-implantes apresentou o maior). Conclusões: Com base nesse estudo de elementos finitos, os resultados mostraram que diferentes designs de expansores palatinos produzem diferentes áreas primárias e níveis de tensão, com deslocamento mínimo das suturas craniofaciais e diferentes expansões esqueléticas em forma de V.

ABSTRACT

Osteoporosis (OP) is a common bone disease affecting the quality of life and causing huge medical burden to the patients and society. The occurrence of OP is mainly caused by excessive bone resorption and insufficient bone formation, which are directly influenced by external calcium ion balance. Calcium imbalance can impair bone integrity, reduce the calcium supply to the bone, and lower the calcium content in the bone, thus triggering OP. Drugs are the main anti-OP therapy in modern medicine, which, however, may cause adverse reactions and drug dependence. Chinese medicines have good clinical effects and high safety in treating OP, being suitable for long-term use. Recent studies have shown that Chinese medicines can alleviate estrogen deficiency, regulate bone cell and calcium metabolism, which is crucial for the formation and development of OP. The transient receptor potential cation channel superfamily V members 5 and 6 (TRPV5 and TRPV6, respectively) affect bone homeostasis by mediating the transmembrane calcium ion transport in the intestine (TRPV6) and kidney (TRPV5). Therefore, TRPV5/6 is one of the key targets to understand the anti-OP mechanisms of the effective parts of Chinese medicines, which is worthy of further study. This paper summarizes the research results about the anti-OP effects of Chinese medicines in the last two decades, especially the mechanism of regulating calcium metabolism, aiming to provide new ideas for the basic research, clinical application, and drug development of OP treatment.

ABSTRACT

Objetivo: El propósito de este estudio fue observar el efecto del uso de L-PRF en defectos infraoseos de pacientes con periodontitis en estadios avanzados. Métodos: Se incluyeron 32 defectos infraoseos de 12 pacientes con diagnóstico de Periodontitis estadio III y IV (Workshop 2018). Se realizó raspaje a campo abierto con colocación de membrana de L-PRF. Se incluyeron defectos infraóseos de 1-2-3 paredes y cráter óseo. Se registró la profundidad de sondaje (PS), nivel de inserción clínica (NIC), índice de placa (IP) e índice de sangrado (IS). Se realizaron radiografías periapicale digitales antes de la cirugía y al cuarto mes para observar el llenado óseo. Resultados: De los 32 defectos el 75 % mostró disminución de la profundidad de sondaje (PS) y el 66 % mejoro el nivel de inserción clínica (NIC). Se realizó un análisis de correlación pre y posquirúrgico en PS: MV (p = 0,02), MP/L (p = 0,00), DP/L (p = 0,00) y V (p =0,00). El porcentaje de llenado óseo fue de 62,96 % (DS± 3,88). Conclusiones: La mayoría de los defectos infraóseos mostraron radiográficamente llenado óseo parcial o total con el uso de membranas L-PRF. Además, se mejoraron los parámetros clínicos de profundidad de sondaje y nivel de inserción clínica.


Objective: The purpose of this study was to observe the effect of L-PRF (Leuko- cyte-Platelet Rich Fibrin) usage in intraosseous defects in patients with advanced-stages of periodontitis. Methods: Thirty-two intraosseous defects in 12 patients diagnosed with stage III and IV periodontitis (Workshop 2018) were included in the study. Open flap debridement was performed with the placement of L-PRF membranes. Included defects consisted of 1-2-3 wall defects and osseous craters. Parameters such as probing depth (PD), clinical attachment level (CAL), plaque index (PI), and bleeding index (BI) were recorded. Digital periapical radiographs were taken before surgery and at the fourth month to assess bone fill. Results: Out of the 32 defects, 75% showed a reduction in probing depth (PD), and 66% showed improvement in clinical attachment level (CAL). Pre- and post-surgical correlation analysis was performed for PD: MV (p = 0.02), PI/L (p = 0.00), BI/L (p = 0.00), and CAL (p = 0.00). The percentage of bone fill was 62.96% (±3.88 SD). Conclusion: The majority of intraosseous defects exhibited partial or complete radiographic bone fill with the use of L-PRF membranes. Furthermore, clinical parameters such as probing depth and clinical attachment level improved.

SUMMARY: In the study, it was aimed to predict sex from hand measurements using machine learning algorithms (MLA). Measurements were made on MR images of 60 men and 60 women. Determined parameters; hand length (HL), palm length (PL), hand width (HW), wrist width (EBG), metacarpal I length (MIL), metacarpal I width (MIW), metacarpal II length (MIIL), metacarpal II width (MIIW), metacarpal III length (MIIL), metacarpal III width (MIIIW), metacarpal IV length (MIVL), metacarpal IV width (MIVW), metacarpal V length (MVL), metacarpal V width (MVW), phalanx I length (PILL), measured as phalanx II length (PIIL), phalanx III length (PIIL), phalanx IV length (PIVL), phalanx V length (PVL). In addition, the hand index (HI) was calculated. Logistic Regression (LR), Random Forest (RF), Linear Discriminant Analysis (LDA), K-nearest neighbour (KNN) and Naive Bayes (NB) were used as MLAs. In the study, the KNN algorithm's Accuracy, SEN, F1 and Specificity ratios were determined as 88 %. In this study using MLA, it is understood that the highest accuracy belongs to the KNN algorithm. Except for the hand's MIIW, MIIIW, MIVW, MVW, HI variables, other variables were statistically significant in terms of sex difference.


En el estudio, el objetivo era predecir el sexo a partir de mediciones manuales utilizando algoritmos de aprendizaje automático (MLA). Las mediciones se realizaron en imágenes de RM de 60 hombres y 60 mujeres. Parámetros determinados; longitud de la mano (HL), longitud de la palma (PL), ancho de la mano (HW), ancho de la muñeca (EBG), longitud del metacarpiano I (MIL), ancho del metacarpiano I (MIW), longitud del metacarpiano II (MIIL), ancho del metacarpiano II (MIIW), longitud del metacarpiano III (MIIL), ancho del metacarpiano III (MIIIW), longitud del metacarpiano IV (MIVL), ancho del metacarpiano IV (MIVW), longitud del metacarpiano V (MVL), ancho del metacarpiano V (MVW), longitud de la falange I (PILL), medido como longitud de la falange II (PIIL), longitud de la falange III (PIIL), longitud de la falange IV (PIVL), longitud de la falange V (PVL). Además, se calculó el índice de la mano (HI). Regresión logística (LR), Random Forest (RF), Análisis discriminante lineal (LDA), K-vecino más cercano (KNN) y Naive Bayes (NB) se utilizaron como MLA. En el estudio, las proporciones de precisión, SEN, F1 y especificidad del algoritmo KNN se determinaron en un 88 %. En este estudio que utiliza MLA, se entiende que la mayor precisión pertenece al algoritmo KNN. Excepto por las variables MIIW, MIIIW, MIVW, MVW, HI de la mano, otras variables fueron estadísticamente significativas en términos de diferencia de sexo.

ABSTRACT Previous studies by us demonstrated that the consumption of thermally oxidized oil diet adversely affects body growth, lipid metabolism, bone mass and femur biomechanical competence. Aim: The aim of this study was to evaluate the effects of a diet containing fried sunflower oil on the mandible of growing rats. Materials and Method: Male Wistar rats (21±1 day old) (n=21) were assigned at weaning to one of three diets for 8 weeks: a control diet (C), a diet containing sunflower oil (SFO) or a diet containing sunflower oil that had been repeatedly heated (SFOx); both SFO and SFOx were mixed with commercial rat chow at 13% (w/w). The consistency and viscosity of the 3 diets were similar. Zoometrics and food intake were recorded weekly. At wk=8, mandibular growth was assessed by measurements of anatomical points of cleaned bones, and mandible biomechanical competence was assessed to estimate the structural properties of the bone. Statistical analysis was performed by SPSS v. 20.0. Results: Rats fed SFOx diet attained the lowest final body weight (P=0.0074), mandibular weight (P=0.0001) and mandibular /length (P=0.0002). Load bearing capacity (Wf;N), load of yielding (Wy;N) and stiffness (Wy/dy;N/mm) of the mandible were negatively affected by both sunflower oil diets (fresh and fried) (P=0.001; P=0.002; P=0.003, respectively) though SFOx induced the most significant reduction in Wy/dy (C:44.4(5.4) > SFO:36.1(2.1) > SFOx: 26.3(3.7) N/ mm; P=0.003). The deleterious effect of SFOx on mandibular growth was more accentuated on the posterior part of the bone (C:11.4(0.3)=SFO:11.2(0.2)>SFOx: 10.7(0.2) mm; p=0.0005); the anterior/ posterior ratio (C:1.25(0.02)=SFO:1.27(0.02)<SFOx:1.32(0.03); p=0.0001) indicated that SFOx induced mandibular deformation. Conclusion: Consumption of SFOx diet during growth could affect mandibular morphometric properties and biomechanical competence, in terms of bone stiffness.


RESUMEN En estudios previos hemos demostrado los efectos adversos del consumo de una dieta rica en aceite termooxidado sobre el crecimiento corporal, el metabolismo de los lípidos, la masa ósea y la competencia biomecánica del fémur. Objetivo: El objetivo de este trabajo fue investigar el efecto de una dieta rica en aceite de girasol termooxidado (AGX) sobre los parámetros morfométricos y biomecánicos de la mandíbula de rata en crecimiento. Materiales y Método: Ratas macho Wistar de 22±1 días de edad (n=21) recibieron durante 8 semanas una de 3 dietas: control (C); dieta comercial, una dieta suplementada con aceite de girasol (AG) y una dieta suplementada con AGX. La consistencia y la viscosidad de las dietas fueron similares. Los parámetros zoométricos y el consumo de dieta se registraron semanalmente. A T=8, los animales se eutanasiaron y se obtuvieron las hemimandíbulas. El crecimiento mandibular se estimó por medidas morfométricas entre puntos anatómicos y las propiedades estructurales por biomecánica. El análisis estadístico se realizó por SPSS v. 20.0. Resultados: Las ratas alimentadas con AGX presentaron menor peso corporal final (p=0.0074), peso mandibular (p=0.0001) y longitud mandibular (p=0.0002). Las propiedades estructurales de la mandíbula, Wf (p=0.001), Wy (p=0.002) y Wy/dy (p=0.003), se vieron afectadas negativamente en ratas alimentadas con AG o AGX, respecto a C; pero la rigidez ósea (Wy/dy) en AGX fue significativamente menor (C:44.4(5.4) > SFO:36.1(2.1) > SFOx: 26.3(3.7) N/mm; p=0.003). El efecto deletéreo del AGX sobre el crecimiento mandibular fue más acentuado en la región posterior (C:11.4(0.3)=SFO:11.2(0.2)>SFOx: 10.7(0.2) mm; p=0.0005). La relación anterior/posterior (C=1.25 (0.02); AG= 1.27(0.02) y AGX=1.32(0.03), p=0.001) indica que AGX indujo deformación mandibular. Conclusiones: El efecto adverso del consumo de una dieta rica en AGX durante el crecimiento podría afectar los parámetros morfométricos y la biomecánica ósea en términos de rigidez ósea.

ABSTRACT

El objetivo de esta investigación fue evaluar la densidad de masa ósea (DMO), la situación nutricional, la ingesta de nutrientes y el nivel de actividad física, por medio de un estudio descriptivo, transversal, en una universidad internacional ubicada en Honduras cuyo universo es 376 empleados, con edad de 40 años y más. La muestra de 50 empleados fue estimada usando la fórmula para poblaciones finitas con una probabilidad de 90% y un error de 10%. Se tomaron medidas antropométricas con equipo SECA; densidad mineral ósea con equipo de ultrasonido en radio Sunlight MiniOmni™. Se aplicó un recordatorio de alimentos consumidos en las últimas 24 horas y el cuestionario Internacional de Actividad Física (IPAQ). Los datos se analizaron en EPI INFO v 7.2.5. El promedio de edad fue 48,9 años con 58% de mujeres participantes. El 90% tuvo sobrepeso y obesidad. Se identificaron 14 casos de DMO no normal, de éstos, uno fue osteoporosis (mujer, de 50 años y más); 93% de los casos tuvieron sobrepeso y obesidad, 43% se identificaron en personas de 40 a 49 años y 71% fueron mujeres. La actividad física fue 48% baja. El promedio de calorías consumidas/día/persona fue de 2.517; con 21% de adecuación de vitamina D, 87% de calcio, 275% de fósforo, 166,69 mg de cafeína. Se concluye que se requiere el control del sobrepeso y obesidad, así como el diagnóstico temprano de los cambios en la densidad mineral ósea, particularmente en las mujeres.


The objective of this research was to evaluate bone mass density(BMD), nutritional status, nutrient intake, and physical activity level, through a cross-sectional descriptive study, in an international university located in Honduras with a universe of 376 employees, aged 40 and over. The sample of 50 was estimated with the formula for finite population with a 90% probability and an error of 10%. Anthropometric measurements were taken with SECA equipment; bone mineral density with Sunlight MiniOmni™ radio ultrasound equipment. A 24-hour dietary recall method and the International Physical Activity Questionnaire (IPAQ) were applied. The data was analyzed in EPI INFO v 7.2.5. The average age of the people in the sample was 48,9 years with 58% of women. 90% were overweight and obese. 14 cases of non-normal BMD were identified, of these, one was osteoporosis (woman, 50 years of age and over); 93% of the cases were overweight and obese, 43% were identified in people between 40 and 49 years of age, and 71% were women. Physical activity was 48% low. The average number of calories consumed/day/person was 2.517; 21% adequacy of vitamin D, 87% of calcium, 275% of phosphorus, 166.69 mg of caffeine. It is concluded that control of overweight and obese is required, and the early diagnosis of changes in bone mass density, particularly in women.

ABSTRACT

Introducción: El dengue es considerado un problema de la salud pública con un comportamiento endémico-epidémico en Colombia, por lo cual los programas de vigilancia epidemiológica se enfocan en la prevención y control. Objetivo: Evaluar la comprensión y el comportamiento de la población del municipio de Barbosa (Santander, Colombia) acerca del virus del dengue y el vector por medio de un instrumento de conocimientos, actitudes y prácticas (CAP). Métodos: Se realizó un estudio transversal analítico en el que participaron 93 habitantes del municipio de Barbosa, seleccionados por un muestreo no probabilístico por conveniencia. Se empleó un instrumento de CAP validado. La información fue sistematizada y analizada en el programa SPSS v. 26.0. Resultados: El 78,5 por cientode los encuestados mostraron conocimientos deficientes con un puntaje obtenido inferior a 70 puntos. El 73,1 por ciento de los habitantes presentó actitudes negativas con un resultado igual o inferior a 24 puntos. El 67,7 por ciento de la población presentó buenas prácticas, con un resultado mayor o igual a 14 puntos. Se evidencia que, a menor edad, son mayores las actitudes desfavorables frente a la infección por dengue (OR 2,67 IC 95 por ciento 1,03-7,1). Conclusiones: Educar a la población de manera más efectiva acerca de los conocimientos básicos sobre el dengue sigue siendo una estrategia necesaria, además de crear conciencia de que las prácticas en el hogar se deben ejecutar con la finalidad de eliminar el vector y no solo como mecanismo de limpieza. Se deben implementar diferentes estrategias comunicativas para educar a los ciudadanos en este sentido(AU)


Introduction: Dengue is considered a public health problem with endemic-epidemic behavior in Colombia; therefore, epidemiological surveillance programs are aimed at its prevention and control. Objective: To evaluate the understanding and behavior of the population from the municipality of Barbosa (Santander, Colombia) on dengue virus and its vector through a knowledge, attitudes, and practices (KAP) survey. Methods: An analytical cross-sectional study was conducted which included 93 residents of the municipality of Barbosa, selected based on non-probabilistic and convenience sampling A validated KAP instrument was used. The information was systematized and analyzed in SPSS v. 26.0. The information was systematized and analyzed in SPSS v. 26.0. Results: Among the respondents, 78.5 percent showed deficient knowledge with a score below 70 points. Negative attitudes were found in 73.1 percent of the residents with a score equal to or lower than 24 points. Best practices were reported by 67.7 percent of the population, with a score greater than or equal to 14 points. It is evident that at younger ages, unfavorable attitudes towards dengue infection are higher (OR 2.67 CI 95% 1.03-7.1). Conclusions: Educating the population more effectively about basic knowledge of dengue remains a necessary strategy, in addition to raising awareness that household practices should be carried out to eliminate the vector and not only as a cleaning mechanism. Different communicative strategies should be implemented to educate citizens in this regard(AU)

ABSTRACT

Objetivo: Comparar dos técnicas de cirugía reconstructiva para lesión en miembro inferior con exposición ósea y, a través de ella, diferenciar que la técnica de VAC® (Vacuum Assisted Closure, cierre asistido con presión negativa) es una alternativa con beneficio de recuperación potencial sin alteraciones significativas que pudieran llevar a un compromiso funcional. Materiales y métodos: Estudio de tipo analítico con corte prospectivo, cuantitativo y longitudinal, en el que se desarrolló la terapia con el uso del sistema de VAC® y de colgajo gemelar medial en todos los pacientes de la Clínica Stella Maris que presentaron heridas traumáticas de miembro inferior con exposición ósea de tercio medio tibial durante el periodo 2019. Resultados: Se evidenció que la medición con la escala funcional de la marcha (FAC, por sus siglas en inglés) fue mejor en los pacientes con la técnica de VAC® (dado que el 50 % tiene grado V) respecto a la técnica de colgajo (50 % en grado IV); las diferencias fueron estadísticamente significativas (p < 0,05). Se apreció que el tiempo de cierre fue mayor en la técnica de VAC® debido al proceso de regeneración progresiva hasta llenar o cubrir la zona completa de la lesión; por otro lado, se evidenció la diferencia de la intensidad del dolor posoperatorio entre las dos técnicas: de moderado a intenso con la técnica de colgajo y leve, en su mayoría, con la técnica de VAC®. Conclusiones: El sistema de aspiración VAC® es eficiente para la cobertura ósea en defectos traumáticos del tercio medio tibial anterior, por lo que constituye una alternativa con potencial beneficio de recuperación sin alteración de estructuras anatómicas, ya que brinda mejores resultados funcionales y menores complicaciones. Es una opción útil que actúa de forma segura porque estimula el cierre de la herida y minimiza las necesidades de un tratamiento quirúrgico.


Objective: To compare two reconstructive surgery techniques for lower limb injury with exposed bone and demonstrate that the VAC® (vacuum-assisted closure) negative pressure wound therapy is an alternative for potential recovery showing no significant changes that could lead to functional compromise. Materials and methods: An analytical, prospective, quantitative and longitudinal study conducted with all the patients of Clínica Stella Maris with traumatic injuries of the lower limb and exposure of the middle third of the tibia treated with the VAC® system and the medial calf flap in 2019. Results: The measurement obtained with the functional ambulation categories (FAC) scale showed better results among the patients treated with the VAC® technique (since 50 % got grade V) than those who underwent the flap technique (50 % got grade IV), being the differences statistically significant (p < 0.05). It was observed that the time to closure was longer with the VAC® technique due to the progressive regeneration process consisting of the complete filling or coverage of the lesion area. On the other hand, the difference in the postoperative pain intensity between the two techniques was evident, being moderate to intense with the flap technique and mild, for the most part, with the VAC® technique. Conclusions: The VAC® suction system is effective for bone coverage in traumatic defects of the anterior middle third of the tibia. It is an alternative for potential recovery that does not change the anatomical structures because it provides better functional results and fewer complications. It is a useful and safe option that stimulates wound closure and minimizes the need for surgical treatment.

ABSTRACT

During bone tumor resection, many cases require medial malleolar osteotomy to achieve adequate access to the operative field. Various osteotomy methods have been developed to address this issue, including oblique, transverse, reverse V-shape, and step-cut osteotomies.However, medial malleolar osteotomy has several drawbacks, such as the excessive disruption of the joint surface, unstable screw fixation when fixing the medial malleolus, and iatrogenic medial ankle joint arthritis due to articular displacement during the reduction of the osteotomy site. In addition, there is a possibility of injury to the posterior tibial artery, tibial nerve, or posterior tibialis tendon if the osteotomy range is too aggressive. Therefore, the authors propose a new osteotomy method, which has shown promising clinical results, namely, partial posterior medial malleolar osteotomy. This method minimizes articular involvement and provides adequate access to the operative field during talar body bone tumor resection.

Background@#When performing reduction of zygomatic arch fractures, locating the inward portion of the fracture can be difficult. Therefore, this study investigated the discrepancy between the locations of the depression on the soft tissue and bone and sought to identify how to determine the inward portion of the fracture on the patient’s face. @*Methods@#We conducted a retrospective review of chart with isolated zygomatic arch fractures of type V in the Nam and Jung classification from March 2013 to February 2022. For consistent measurements, a reference point (RP), at the intersection between a vertical line passing through the end point of the root of the ear helix in the patient’s side-view photograph and a transverse line passing through the longest horizontal axis of the external meatus opening, was established. We then measured the distance between the RP and the soft tissue depression in a portrait and the bone depression on a computed tomography (CT) scan. The discrepancy between these distances was quantified. @*Results@#Among the patients with isolated zygomatic arch fractures, only those with a fully visible ear on a side-view photograph were included. Twenty-four patients met the inclusion criteria. There were four types of discrepancies in the location of the soft tissue depression compared to the bone depression: type I, forward and upward discrepancy (7.45 and 3.28 mm), type II, backward and upward (4.29 and 4.21 mm), type III, forward and downward (10.06 and 5.15 mm), and type IV, backward and downward (2.61 and 3.27 mm). @*Conclusion@#This study showed that discrepancy between the locations of the depressions on the soft tissue and bone exists in various directions. Therefore, applying the transverse and vertical distances measured from a bone image of the CT scan onto the patient’s face at the indicated RP will be helpful for predicting the reduction location.

OBJECTIVE@#To investigate the inflammatory effects of Cinobufotalin on monocytes in resting state and macrophages in activated state and its molecular mechanism.@*METHODS@#THP-1 cells were stimulated with Phorbol 12-myristate 13-acetate to induce differentiation into macrophages. Lipopolysaccharides was added to activate macrophages in order to establish macrophage activation model. Cinobufotalin was added to the inflammatory cell model for 24 h as a treatment. CCK-8 was used to detect cell proliferation, Annexin V /PI double staining flow cytometry was used to detect cell apoptosis, flow cytometry was used to detect macrophage activation, and cytometric bead array was used to detect cytokines. Transcriptome sequencing was used to explore the gene expression profile regulated by Cinobufotalin. Changes in the significantly regulated molecules were verified by real-time quantitative polymerase chain reaction and Western blot.@*RESULTS@#1∶25 concentration of Cinobufotalin significantly inhibited the proliferation of resting monocytes(P<0.01), and induced apoptosis(P<0.01), especially the activated macrophages(P<0.001, P<0.001). Cinobufotalin significantly inhibited the activation of macrophages, and significantly down-regulated the inflammatory cytokines(IL-6, TNF-α, IL-1β, IL-8) released by activated macrophages(P<0.001). Its mechanism was achieved by inhibiting TLR4/MYD88/P-IκBa signaling pathway.@*CONCLUSION@#Cinobufotalin can inhibit the inflammatory factors produced by the over-activation of macrophages through TLR4/MYD88/P-IκBa pathway, which is expected to be applied to the treatment and research of diseases related to the over-release of inflammatory factors.

ABSTRACT

PURPOSE@#Intramedullary implants are well accepted fixation of all types of intertrochanteric (IT) fractures, both stable and unstable types. Intramedullary nails have an ability to effectively support the posteromedial part, but fail to buttress the broken lateral wall requiring lateral augmentation. The aim of this study was to evaluate the outcome of proximal femoral nail augmented with trochanteric buttress plate for broken lateral wall with IT fractures, which was fixed to the femur through hip screw and anti-rotation screw nail.@*METHODS@#Of 30 patients, 20 had Jensen-Evan type III and 10 had type V fractures. Patients with IT fracture of broken lateral wall and aged more than 18 years, in whom satisfactory reduction was achieved by closed methods, were included in the study. Patients with pathologic or open fractures, polytrauma, prior hip surgery, non-ambulatory prior to surgery, and those who refused to participate were excluded. The operative time, blood loss, radiation exposure, quality of reduction, functional outcome, and union time were evaluated. All data were coded and recorded in Microsoft Excel spread sheet program. SPSS 20.0 was used for data analysis and normality of the continuous data was checked using Kolmogorv Smirnov test.@*RESULTS@#The mean age of patients in the study was 60.3 years. The mean duration of surgery (min), mean intra-operative blood loss (mL) and mean number of exposures were 91.86 ± 12.8 (range 70 - 122), 144.8 ± 3.6 (range 116 - 208), and 56.6 (range 38 - 112), respectively. The mean union time was 11.6 weeks and the mean Harris hip score was 94.1.@*CONCLUSION@#Lateral trochanteric wall in IT fractures is significantly important, and needs to be reconstructed adequately. Nail-plate construct of trochanteric buttress plate fixed with hip screw and anti-rotation screw of proximal femoral nail can be successfully used to augment, fix or buttress the lateral trochanteric wall giving excellent to good results of early union and reduction.

ABSTRACT

Osteoclast (OC) is multinucleated, bone-resorbing cells originated from monocyte/macrophage lineage of cells, excessive production and abnormal activation of which could lead to many bone metabolic diseases, such as osteoporosis, osteoarthritis, etc. Autophagy, as a highly conserved catabolic process in eukaryotic cells, which plays an important role in maintaining cell homeostasis, stress damage repair, proliferation and differentiation. Recent studies have found that autophagy was also involved in the regulation of osteoclast generation and bone resorption. On the one hand, autophagy could be induced and activated by various factors in osteocalsts, such as nutrient deficiency, hypoxia, receptor activator of nuclear factor(NF)-κB ligand(RANKL), inflammatory factors, wear particles, microgravity environment, etc, different inducible factors, such as RANKL, inflammatory factors, wear particles, could interact with each other and work together. On the other hand, activated autophagy is involved in regulating various stages of osteoclast differentiation and maturation, autophagy could promote proliferation of osteoclasts, inhibiting apoptosis, and promoting differentiation, migration and bone resorption of osteoclast. The classical autophagy signaling pathway mediated by mammalian target of rapamycin complex 1(mTORC1) is currently a focus of research, and it could be regulated by upstream signalings such as phosphatidylinositol 3 kinase(PI-3K)/protein kinase B (PKB), AMP-activated protein kinase(AMPK). However, the paper found that mTORC1-mediated autophagy may play a bidirectional role in regulating differentiation and function of osteoclasts, and its underlying mechanism needs to be further ciarified. Integrin αvβ3 and Rab protein families are important targets for autophagy to play a role in osteoclast migration and bone resorption, respectively. In view of important role of osteoclast in the occurrence of various bone diseases, it is of great significance to elucidate the role of autophagy on osteoclast and its mechanism for the treatment of various bone diseases. The autophagy pathway could be used as a new therapeutic target for the treatment of clinical bone diseases such as osteoporosis.

OBJECTIVE@#To evaluate the effectiveness of neurovascular staghorn flap for repairing defects in fingertips.@*METHODS@#Between August 2019 and October 2021, a total of 15 fingertips defects were repaired with neurovascular staghorn flap. There were 8 males and 7 females with an average age of 44 years (range, 28-65 years). The causes of injury included 8 cases of machine crush injury, 4 cases of heavy object crush injury, and 3 cases of cutting injury. There were 1 case of thumb, 5 cases of index finger, 6 cases of middle finger, 2 cases of ring finger, and 1 case of little finger. There were 12 cases in emergency, and 3 cases with finger tip necrosis after trauma suture. Bone and tendon exposed in all cases. The range of fingertip defect was 1.2 cm×0.8 cm to 1.8 cm×1.5 cm, and the range of skin flap was 2.0 cm×1.5 cm to 2.5 cm×2.0 cm. The donor site was sutured directly.@*RESULTS@#All flaps survived without infection or necrosis, and the incisions healed by first intention. All patients were followed up 6-12 months, with an average of 10 months. At last follow-up, the appearance of the flap was satisfactory, the wear resistance was good, the color was similar to the skin of the finger pulp, and there was no swelling; the two-point discrimination of the flap was 3-5 mm. One patient had linear scar contracture on the palmar side with slight limitation of flexion and extension, which had little effect on the function; the other patients had no obvious scar contracture, good flexion and extension of the fingers, and no dysfunction. The finger function was evaluated according to the total range of motion (TAM) system of the Hand Surgery Society of Chinese Medical Association, and excellent results were obtained in 13 cases and good results in 2 cases.@*CONCLUSION@#The neurovascular staghorn flap is a simple and reliable method to repair fingertip defect. The flap has a good fit with the wound without wasting skin. The appearance and function of the finger are satisfactory after operation.

ABSTRACT

Introducción: La trombocitemia esencial es una neoplasia mieloproliferativa crónica caracterizada por una trombocitosis mantenida en sangre periférica y una hiperplasia de megacariocitos maduros en médula ósea, con características histológicas y moleculares específicas. Objetivo: Caracterizar los pacientes adultos con trombocitemia esencial en el Instituto de Hematología e Inmunología. Métodos: Se realizó un estudio observacional, descriptivo, longitudinal y retrospectivo. El universo estuvo conformado por 40 pacientes adultos con trombocitemia esencial diagnosticados y tratados con trombocitemia esencial, en el Instituto de Hematología e Inmunología desde enero del 2010 hasta enero del 2020. Los datos se almacenaron en una base de datos confeccionada con el programa SPSS v.25.0 para Windows, a partir de la cual fueron procesados. Resultados: El promedio de edad al diagnóstico fue 52,2 años y una mediana de 51 años. El 80 % correspondió al sexo femenino y el 62,5 % de los pacientes tenían el color de la piel blanca. El 85 % de los pacientes presentaron recuento de plaquetas en el rango entre 450-1500 ( 109/L. El 52,1 % de los casos presentó aumento de la enzima lactato deshidrogenasa y la mutación del JAK2V617F representó el 67,5 %. La supervivencia global en años fue 35,5 y la supervivencia libre de enfermedad fue 32,5 años. Conclusiones: Los pacientes adultos con trombocitemia esencial tienen una supervivencia global y libre de enfermedad significativamente elevada, con baja incidencia de eventos trombóticos y el tratamiento empleado de primera línea se relacionó con la respuesta hematológica estable de los pacientes.


Introduction: Essential thrombocythemia is a chronic myeloproliferative neoplasm characterized by thrombocytosis maintained in peripheral blood and hyperplasia of mature megakaryocytes in bone marrow, with specific histological and molecular characteristics. General: To characterize of adult patients with essential thrombocythemia in the Institute of Hematology and Immunology. Methods: An observational, descriptive, longitudinal and retrospective study was conducted. The universe consisted of 40 adult patients diagnosed and treated with essential thrombocythemia at the Institute of Hematology and Immunology from January 2010 to January 2020. The data obtained were stored in a database made with the program SPSS v 25.0 for Windows, from which they were processed. Results: The average age at diagnosis was 52,2 years and a median of 51 years. 80% occurred in females and 62,5% of patients were white. 85% of the patients had platelet counts in therange between 450-1500 ( 109/L. An increase in the enzyme lactate dehydrogenase was observed in 52,1% of the cases, and the JAK2V617Fmutationaccountedfor 67,5%. The overall survival in years was 35,5 and the disease-free survival was 32.5 years. Conclusions: Adult patients with essential thrombocythemia have significantly increased overall and disease-free survival, with low incidence of thrombotic events and first-line treatment was associated with stable hematological response of patients.

ABSTRACT Objective: To compare the initial stress distribution and displacement on mandibular dentition using extra and inter-radicular mini-implants for arch distalization, by means of finite element analysis. Methods: For this study, two finite element models of the mandible were designed. The models consisted of periodontal ligament (PDL) and alveolar bone of all teeth until second molars. In the Case 1, bilateral extra-radicular buccal-shelf stainless steel mini-implants (10.0-mm length; 2.0-mm diameter) were placed between first and second permanent molars. In the Case 2, bilateral inter-radicular stainless steel mini-implants (10.0-mm length; 1.5-mm diameter) were placed between second premolar and first permanent molar. Power hook was attached between canine and first premolar at a fixed height of 8mm. In the two cases, 200g of distalization force was applied. ANSYS v. 12.1 software was used to analyze and compare von Mises stress and displacement in the mandibular dentition, PDL and bone. Results: Higher stresses were observed in mandibular dentition with the inter-radicular implant system. The amount of von Mises stress was higher for cortical bone (85.66MPa) and cancellous bone (3.64MPa) in Case 2, in comparison to cortical bone (41.93MPa) and cancellous bone (3.43MPa) in Case 1. The amount of arch distalization was higher for mandible in Case 1 (0.028mm), in comparison to Case 2 (0.026mm). Conclusion: Both systems were clinically safe, but extra-radicular implants showed more effective and controlled distalization pattern, in comparison to inter-radicular implants, in Class III malocclusion treatment.


RESUMO Objetivo: Comparar a distribuição da tensão inicial e o deslocamento na dentição inferior usando mini-implantes extra e inter-radiculares para distalização da arcada, por meio da análise de elementos finitos. Métodos: Dois modelos de elementos finitos da mandíbula foram criados, os quais consistiram de ligamento periodontal (PDL) e osso alveolar de todos os dentes até os segundos molares. No Caso 1, mini-implantes extra-radiculares de aço inoxidável (10,0 mm de comprimento; 2,0 mm de diâmetro) foram colocados bilateralmente na buccal-shelf entre o primeiro e o segundo molares permanentes. No Caso 2, mini-implantes de aço inoxidável inter-radiculares (comprimento de 10,0 mm; diâmetro de 1,5 mm) foram colocados bilateralmente entre o segundo pré-molar e o primeiro molar permanentes. Um Power hook foi preso entre o canino e o primeiro pré-molar a uma altura fixa de 8mm. Nos dois casos, foi aplicada força de distalização de 200g. O software ANSYS v. 12.1 foi usado para analisar e comparar a tensão de von Mises e o deslocamento na dentição inferior, ligamento periodontal e osso. Resultados: Maiores tensões foram observadas na dentição inferior com o sistema de implantes inter-radiculares. A quantidade de tensões de von Mises foi maior para osso cortical (85,66MPa) e osso esponjoso (3,64MPa) no Caso 2, em comparação com osso cortical (41,93MPa) e osso esponjoso (3,43MPa) no Caso 1. A quantidade de distalização da arcada inferior foi maior no Caso 1 (0,028 mm), em comparação com o Caso 2 (0,026 mm). Conclusão: Ambos os sistemas foram clinicamente seguros, mas os implantes extra-radiculares mostraram um padrão de distalização mais eficaz e controlado, em comparação com os implantes inter-radiculares, para tratamento da má oclusão de Classe III.

ABSTRACT

【Objective】 To investigate the preventive effects of early apoptotic splenic mononuclear cells induced by extracorporeal photopheresis (ECP) on acute graft versus host disease (aGVHD) in mice and explore the underlying mechanisms. 【Methods】 1) Splenic mononuclear cells were extracted from C57BL/6 mice and treated with different concentrations of 8-MOP (50 ng/mL, 100 ng/mL, 200 ng/mL, 300 ng/mL, 600 ng/mL). After treatment, irradiate the cells with 2 J/cm2 of ultraviolet light. Then, use the Annexin V-FITC/PI apoptosis detection kit to assess the early apoptosis rate of the cells and determine the optimal concentration of 8-MOP for the experiment.2) There were 35 SPF-grade female BALB/C mice (H-2Kd) aged 6-8 weeks. After whole-body irradiation with 8Gy X-rays, the mice were divided into five groups: sham irradiation group received intravenous infusion of 0.2 mL of normal saline, the syngeneic bone marrow transplantation group received intravenous infusion of 0.2 mL of BALB/C mouse bone marrow nucleated cell suspension (including a cell count of 1×107), the allogeneic bone marrow transplantation group received intravenous infusion of 0.2 mL of C57BL/6 mouse bone marrow nucleated cell suspension (including a cell count of 1×107), the aGVHD group received intravenous injection of a mixture of C57BL/6 mouse bone marrow nucleated cells (including a cell count of 1×107) and splenic mononuclear cells (including a cell count of 1×107) in 0.2 mL, the ECP prevention group received pre-transplant intravenous infusion of 0.2 mL of ECP-treated splenic mononuclear cells of C57BL/6 mice (including a cell count of 1×107 ) 48 hours before transplantation, and on the day of transplantation, intravenous injection of a mixture of C57BL/6 mouse bone marrow nucleated cells (including a cell count of 1×107) and splenic mononuclear cells (including a cell count of 1×107 ) in 0.2 mL.The preventive effects of ECP on aGVHD were observed, and the concentrations of IFN-γ, IL-2, TNF, IL-4 and IL-6 in mouse serum were measured using CBA. Th1 cell counts were determined by flow cytometry. 【Results】 Different concentrations of 8-MOP (50 ng/mL,100 ng/mL, 200 ng/mL, 300 ng/mL, 600 ng/mL) were used to treat mouse splenic mononuclear cells. The early apoptosis rates (%), observed after treatment were as follows: (14.18±0.865) vs (16.76±0.407) vs (18.83±0.404) vs (19.27±0.404) vs (14.5±0.529). The appropriate concentration of 8-MOP was determined to be 200 ng/mL.In vivo experiment, the results showed that the aGVHD group had decreased survival rate, reduced body weight, and increased clinical scores compared to the syngeneic and allogeneic bone marrow transplantation groups (P<0.01), and the chimerism of bone marrow cells in mice after transplantation was over 90%. ECP significantly improved the survival rate of mice after transplantation, reduced clinical scores (P<0.05), and decreased the concentrations of Th1 cell cytokines in serum (P<0.05) and the counts of Th1 cells in the spleen (P<0.05). 【Conclusion】 ECP-induced early apoptotic single nuclear cells from the spleen can prevent the occurrence of aGVHD by reducing the Th1 response in mouse.

Introduction: Failure of ossification in the vertical direction between the two halves of the frontal bone is called a metopic suture; this suture is present in between the superciliary arch and tubers of the frontal bone so it is also called a median frontal suture. At birth, the frontal bone present is two half, in the 1st year they contact each other, and within the 7th to 8th year, they fused together. Sometimes, bones may not be fused completely and it becomes metopic fontanelle. MaterialsandMethods: The goal of the current study was to determine the prevalence of metopic suture in adult human skulls in Uttar Pradesh. One hundred and fifty macerated skulls of undetermined age and sex were used for this. The anatomical departments of Teerthankar Mahaveer Medical College and Research Center (Moradabad), King George Medical College (Lucknow), SRMS Institute of Medical Science (Bareilly), Govt. Medical College Kannauj, and Govt. Medical College Saharanpur provided these skulls. Results: This study has found the incidence of metopic sutures of about 12.90%. The two types of metopic sutures were found in this study, namely complete metopic suture 4.52% and incomplete metopic suture 8.38% in which found 0.64% “V” shape metopic suture. Conclusion: Themorphological study of metopic sutures on adult crania is useful for anatomists, experts in forensic medicine, and neurologists for performing surgical procedures in this area

A primeira parte do trabalho avaliou, através de uma revisão sistemática de estudos in vitro, a aplicabilidade da fotobiomodulação como uma ferramenta auxiliar na engenharia de tecidos. De 8373 estudos inicialmente identificados a partir das estratégias de busca, dez artigos atingiram os critérios de inclusão para análise. Os dados obtidos na maioria dos estudos revisados indicaram que a laserterapia de baixa intensidade (LBI) pode aumentar a proliferação e diferenciação de células cultivadas na superfície dos biomateriais. Na segunda parte do trabalho foi avaliado o efeito da LBI na dose de 4 J/cm2 na proliferação de osteoblastos (OFCOL II) cultivados na superfície de arcabouços poliméricos tridimensionais (3D) de ácido polilático (PLA) e de PLA associado a quitosana (PLA/Q) produzidos pela técnica de fiação por sopro em solução. O ensaio do Alamar Blue demonstrou que as células OFCOL II cultivadas sobre os arcabouços 3D de PLA e irradiadas apresentaram uma maior atividade proliferativa quando comparadas aos grupos não irradiados no intervalo de 72 h. Além disso, as células OFCOL II cultivadas sobre arcabouços de PLA/Q também apresentaram uma maior atividade proliferativa em 24 h. A análise pela microscopia eletrônica de varredura (MEV) mostrou que os osteoblastos se encontravam ancorados em concavidades das fibras nos arcabouços examinados. Concluiu-se que o modelo proposto apresentou um potencial para estudos na área da engenharia tecidual óssea. Na terceira parte do trabalho foi avaliada a influência da LBI infravermelha (IV) e vermelha (V) em diferentes dosagens (1 J/cm², 4 J/cm² e 6 J/cm²) na proliferação e viabilidade das células OFCOL II. O ensaio do Alamar Blue mostrou diferenças significativas (p<0,05) na atividade mitocondrial do grupo IV utilizando a dose de 1 J/cm2 e 4 J/cm2, nos intervalos de 24 e 48 h. Já o ensaio do Live/Dead evidenciou que a LBI induziu aumento da viabilidade celular no grupo IV na dose de 4 J/cm2, quando comparada com os demais grupos. Em conjunto, os resultados sugerem que a LBI pode promover bioestimulação in vitro de osteoblastos, inclusive quando cultivados na superfície de arcabouços poliméricos 3D, representando assim uma ferramenta promissora nas técnicas de engenharia tecidual óssea (AU).


The first part of the work evaluated, through a systematic review of in vitro studies, the applicability of photobiomodulation as an auxiliary tool in tissue engineering. Of 8373 studies initially identified from the search strategies, ten articles met the inclusion criteria for analysis. Data obtained from most of the reviewed studies indicated that low-intensity laser therapy (LLLT) could increase the proliferation and differentiation of cells cultured on the surface of biomaterials. The second part of the work evaluated the effect of LLLT at a dose of 4 J/cm² on the proliferation of osteoblasts (OFCOL II) cultivated on the surface of threedimensional (3D) polymer scaffolds of polylactic acid (PLA) and PLA associated with chitosan (PLA/Q) produced by the solution blow spinning technique. The Alamar Blue assay demonstrated that OFCOL II cells cultured on 3D PLA scaffolds and irradiated showed more significant proliferative activity when compared to non-irradiated groups within 72 h. Furthermore, OFCOL II cells cultured on PLA/Q scaffolds showed higher proliferative activity at 24 h. Analysis by scanning electron microscopy (SEM) showed that the osteoblasts were anchored in the concavities of the fibers of the examined scaffolds. It was concluded that the proposed model showed potential for studies in the field of bone tissue engineering. The third part of the work evaluated the influence of infrared (IR) and red (R) laser therapy at different dosages (1 J/cm², 4 J/cm², and 6 J/cm²) on the proliferation and viability of OFCOL II cells. The Alamar Blue assay showed significant differences (p<0.05) in the mitochondrial activity of group IR using the dose of 1 J/cm² and 4 J/cm² at 24 and 48 h. The Live/Dead assay showed that LLLT induced an increase in cell viability in the IR group at a dose of 4 J/cm² compared to the other groups. Taken together, the results suggest that LLLT can promote in vitro biostimulation of osteoblasts, even when cultivated on the surface of 3D polymeric scaffolds, thus representing a promising tool in bone tissue engineering techniques (AU).

ABSTRACT

O nervo mandibular V3 é a terceira divisão do nervo trigêmeo, apresenta fibras motoras e sensitivas, sendo a ramificação mista deste nervo. Seus ramos eferentes são responsáveis pela motricidade dos músculos mastigatórios, inerva os músculos milo-hióideo, ventre anterior do digástrico, tensor do véu palatino e tensor do tímpano. Suas fibras aferentes são responsáveis pela sensibilidade da região inferior da face e cavidade oral, dentes inferiores, parte da língua e propriocepção da cápsula da articulação temporomandibular ATM. O objetivo deste estudo é explanar as possíveis lesões no nervo mandibular, suas ramificações e estruturas adjacentes no caso de fraturas na mandíbula, colaborando com informações detalhadas para posterior estudo de acadêmicos e cirurgiões dentistas. Foi realizada uma pesquisa de campo, exploratória e descritiva, com abordagem quanti-qualitativa, para descrição e análise do trajeto do V3 até a sua chegada no osso mandibular, assim como de estruturas que compõem a topografia adjacente a mesma utilizando uma hemiface intacta de um cadáver do sexo masculino no Laboratório de Anatomia Humana das Faculdades Nova Esperança, na cidade de João Pessoa. Devido à proximidade do nervo mandibular com o processo coronoide, fraturas nesta parte do osso mandíbula, podem ocasionar lesões no V3, além de traumas no osso temporal, e consequentemente de todo o gânglio trigeminal. Além destas, estruturas adjacentes como a glândula parótida, artérias maxilar e facial, ATM, e os nervos alveolar inferior, mentual e lingual, podem ser lesionadas em uma fratura mandibular. As lesões nestes nervos, podem resultar em redução grave da qualidade de vida e dor crônica, gerando desconfortos para o paciente. Diante do exposto fica evidente a importância de conhecer a anatomia topográfica da mandíbula e suas estruturas vasculonervosas, seu trajeto e a localização. Houve dificuldade na discussão devido à escassez da literatura em relação à temática proposta. Dessa forma, é necessário motivar novos estudos sobre a temática a fim de ampliar o conhecimento dos profissionais de saúde e estimular novas técnicas para diagnóstico precoce e melhorar os resultados terapêuticos, impactando positivamente na sobrevida de pacientes com fraturas de mandibulares(AU)


The mandibular nerve V3 is the third division of the trigeminal nerve, has motor and sensory fibers, being the mixed branch of this nerve. Its efferent branches are responsible for the motricity of the masticatory muscles, innervating the mylohyoid muscles, anterior belly of the digastric, soft palate tensor and tympanic tensor. Its afferent fibers are responsible for the sensitivity of the lower face and oral cavity, lower teeth, part of the tongue and proprioception of the capsule of the temporomandibular joint TMJ. The aim of this study is to explain the possible injuries to the mandibular nerve, its ramifications and adjacent structures in the case of mandible fractures, collaborating with detailed information for further study by academics and dentists. A field research, exploratory and descriptive, with a quantitative-qualitative approach, was carried out to describe and analyze the path of the V3 until its arrival in the mandibular bone, as well as structures that make up the topography adjacent to it using na intact hemiface of a male cadaver at the Human Anatomy Laboratory of Faculdades Nova Esperança, in João Pessoa city. Due to the proximity of the mandibular nerve with the coronoid process, fractures in this part of the mandible bone can cause injuries to the V3, in addition to trauma to the temporal bone, and consequently to the entire trigeminal ganglion. In addition to these, adjacent structures such as the parotid gland, maxillary and facial arteries, TMJ, and the inferior alveolar, mental and lingual nerves can be injured in a mandibular fracture. Damage to these nerves can result in severely reduced quality of life and chronic pain, causing discomfort for the patient. Given the above, the importance of knowing the topographic anatomy of the mandible and its vascular-nervous structures, its path and location is evident. There was difficulty in the discussion due to the scarcity of literature regarding the proposed theme. Thus, it is necessary to motivate further studies on the subject in order to expand the knowledge of health professionals and encourage new techniques for early diagnosis and improve therapeutic results, positively impacting the survival of patients with mandibular fractures(AU)
